Northumberland Family Health Team (NFHT) – Primary Care, Illness Prevention and Chronic Disease Management.

NFHT is here to partner with you, our rostered patients, by working with your family doctor and offering programs and services focused on prevention of illness, promoting health and wellness and treating and managing health conditions. Our partner medical clinics include Cobourg Clinic, Cobourg Health Centre, and Cobourg Medical Centre.

We serve over 26,000 patients with 21 local family doctors and a host of other care providers including Nurse Practitioners, Registered Nurses, Dietitians, an Occupational Therapist, a Chiropodist (Foot Specialist), Social Workers, a Pharmacist and a Physiotherapist. Our health care providers work in teams with patients and their caregivers, to provide programs and services such as Chronic Disease Management (for people living with Hypertension, Chronic Obstructive Pulmonary Disease or other chronic conditions), Healthy Living and Prevention (including immunizations, cancer screening), mental health counseling and workshops, seniors wellness/ Primary Care Memory Clinic, Footcare, Clinical Nutrition and more. From understanding and managing a chronic condition like hypertension or COPD to working through mental health concerns, we provide screening, education, counseling and support to our patients.

Patient Information
You are considered a patient of the Northumberland Family Health Team (NFHT) if you are enrolled with one of the 21 family doctors listed on our Hours of Operation Page. Your family doctor will now have the opportunity to work with other healthcare professionals to provide a group approach to your primary healthcare needs.

As a patient, you now have access to many no-cost services and programs to help you meet your healthcare goals. You will continue to see your family doctor on a regular basis, however, other members of NFHT may assist your doctor in providing primary healthcare services. NFHT health care providers work together with your family physician/NP, including documenting your care into a shared medical record.

Example: You may have an appointment with your family doctor where it is determined that you have high blood pressure. Your family doctor may prescribe you medications and then refer you to the NFHT “Hypertension Program” where you will work with a team that could include a Registered Nurse who will help you to understand what is contributing to your high blood pressure, a Pharmacist who will assess and adjust your medications, a physiotherapist who will work with you on an exercise program and/or a Mental Health Clinician to discuss stress management. This team will work with you to help identify goals and provide support in a way that works for you. All information is recorded in a shared medical record, with communication back to your family doctor as needed.

You will continue to visit your family doctor/nurse practitioner at their current office location.

If you are referred by your doctor or nurse practitioner to see one of the health care professionals, your doctor will send a referral to the NFHT main office. The administrative staff at the NFHT main office will contact you to schedule an appointment.

All healthcare professional visits will occur at the NFHT main office on the lower level of the Northumberland Mall.
