Family Health Team

Family Health Teams are patient-centred primary care organizations that offer patients access to a number of primary healthcare services from a variety of team members, beyond a family doctor.

Northumberland Family Health Team (NFHT) serves over 26,000 patients of 20 local family doctors who work together with other healthcare professionals to coordinate the highest possible quality of care for their enrolled patients. Our affiliated medical clinics include Cobourg Clinic, Cobourg Health Centre, and Cobourg Medical Centre.

NFHT offers primary and preventative health care services to patients of the participating doctors.

Our health care professionals include: Nurse Practitioners, Registered Nurses, Dietitians, an Occupational Therapist, a Chiropodist, Social Workers, a Mental Health Clinician, a Pharmacist, and a Physiotherapist. Each healthcare professional has special training, skills, and experience and will work with family doctors to provide primary health care to patients and their families.

NFHT emphasizes health promotion and improved management of chronic disease through treatment and monitoring as well as supporting their patients in improving self-management skills. Programs and services are being developed based on a strategic review of the healthcare needs of the patients and the community needs.

• Call us. If you have a new or ongoing health concern or question, your family doctor and health team want to hear from you.
• Wear a mask. Masking is required at all of our sites.
• Prescriptions. If you need a prescription refill, please request this through your pharmacy.
For non-urgent concerns, or reasons other than appointment booking, please call in the afternoon.

Patient Information
You are considered a patient of the Northumberland Family Health Team (NFHT) if you are enrolled in one of the 20 family doctors listed on our Hours of Operation Page. Your family doctor will now have the opportunity to work with other healthcare professionals to provide a group approach to your primary healthcare needs.

As a patient, you now have access to many new no-cost services and programs to help you meet your healthcare goals. You will continue to see your family doctor on a regular basis, however, other members of the NFHT may assist your doctor in providing primary healthcare services.

Example: You may have an appointment with your family doctor where it is determined that you have a respiratory problem (eg. Asthma). Your family doctor may then refer you to the NFHT “Healthy Lung Program” where you will first see the Registered Nurse at your doctor’s office and then you will have an appointment with the Respiratory Therapist within the NFHT offices. The Respiratory Therapist will then communicate all pertinent health information back to your family doctor.

You will continue to visit your family doctor at their current office location.

If you are referred by your doctor to see one of the health care professionals your doctor will send a referral to the NFHT main office. The administrative staff at the NFHT main office will contact you to schedule an appointment.

All healthcare professional visits will occur at the NFHT main office on the lower level of the Northumberland Mall.
