Northumberland Family Health Team … A Team-based Approach To Your Care

Since 2010, the Northumberland Family Health Team has provided a team-based approach to primary care, prevention and chronic disease management. By partnering with family (primary care) doctors, our patients have access to interprofessional team of healthcare providers to support their health and wellness needs, in every stage of life.

In addition to your relationship with your family doctor, patients benefit from the expertise of Nurse Practitioners, Registered Nurses, a Clinical Pharmacists, Registered Dietitians, a Physiotherapist, an Occupational Therapist, Social Workers and a Chiropodist. They work together in teams to provide programs and services such as Chronic Disease Management (for people living with Hypertension, Chronic Obstructive Pulmonary Disease or other chronic diseases), Healthy Living and Prevention, mental health counselling and workshops, Seniors Wellness/Primary Care Memory Clinic, Footcare, Clinical Nutrition and many more.

By connecting patients to a full team of specialists, we help define your specific needs and goals. Then, we will help put a program in place to meet them. This leads to a more prevention-based model in addition to addressing symptoms and the best possible recovery. Our health professionals work together, with family doctors, as a team and in a shared medical record to ensure care is comprehensive and coordinated.

MISSION

In an integrated and collaborative manner, the Northumberland Family Health Team will efficiently provide high quality, accessible patient centered care that offers a comprehensive range of core and targeted services aimed at managing and preventing disease and improving the health status of the population.

STRATEGIC PRIORITIES

  1. Provide timely access to primary health care allowing residents the opportunity to receive the appropriate services now and into the future.
  2. Develop and deliver collaborative Chronic Disease Management and Health Promotion programs which are patient-centered, focusing on those with the highest need.
  3. Create a culture of inter-professional practice, collaboration and care that enables quality patient care and a healthy workplace.
  4. Work collaboratively within hospital, community health and social services partners to improve the range, quality, efficiency and collaboration of health care services in West Northumberland.
  5. Develop a strong model of governance that ensures effective controls and procedures are in place to mitigate ongoing risks.
