You are considered a patient of the Northumberland Family Health Team (NFHT) if you are enrolled with one of the family doctors listed on our Hours of Operation Page. Your family doctor works with our other NFHT healthcare professionals to provide a group approach to your primary healthcare needs.

What does the Northumberland Family Health Team mean to YOU – the patient?

As a patient, you have access to many no-cost services and programs to help you meet your healthcare goals. You will continue to see your family doctor on a regular basis, however, other members of the NFHT team may assist your doctor in providing primary healthcare services.

Example: You may have an appointment with your family doctor where it is determined that you have a respiratory problem (eg. COPD). Your family doctor may then refer you to the NFHT “Healthy Lung Program” where you will first see the Registered Nurse at your doctor’s office and then you will have an appointment with the healthcare provider in the COPD program at an NFHT office. Any healthcare provider at NFHT will be communicating all pertinent health information back to your family doctor.

Referral Process and Location of Services and Programs

You will continue to visit your family doctor at their current office location.

If you are referred by your doctor to see one of the NFHT team’s health care professionals your doctor will send a referral to the NFHT main office. The administrative staff at the NFHT main office will contact you to schedule an appointment. You may also self-refer to most of our NFHT programs by calling 289-252-2139.

All healthcare professional visits will occur at the NFHT main office on the lower level of the Northumberland Mall.
