Assessment, treatment, monitoring, and support for people with Chronic Obstructive Pulmonary Disease (COPD).

People with a suspected or confirmed diagnosis of COPD.

• Comprehensive respiratory assessment and/or facilitation of external testing as required
• Health promotion and prevention related to lung health including referral to smoking cessation support, pulmonary rehab, and vaccinations as needed
• Development of an Action Plan to support self-management of COPD and acute exacerbations
• Individual and group monitoring and health teaching

• Improved respiratory health and overall quality of life
• Improved confidence and ability to manage COPD and its symptoms

• Physiotherapist/Certified Respiratory Educator
• Registered Nurse
• Dietitian
• Pharmacist
• Mental Health Clinician

Physician, Nurse Practitioner, or NFHT Clinician referral

You can book an appointment online click here. This booking is available to any patient of the Northumberland Family Health Team with a diagnosis of Chronic Obstructive Pulmonary Disease (COPD). This program can support you with the following:

  • Understanding what COPD is
  • Reviewing how COPD is impacting you on a daily basis
  • Learning how to manage your COPD so that you can live your best life
  • Learning how to monitor your symptoms, notice when they are changing, and how to manage potential flare-ups.
  • Learning about available resources to help you manage your COPD

What you can expect:

  • A thorough review of your health history including your lung medications, any relevant breathing tests
  • An assessment of your symptoms and how they affect your day to days
  • A physical assessment including vitals and physical activity status
  • Recommendations on next steps you can do to better manage your COPD
  • A report back to your Family Physician with any recommended changes to your care plan or requests for any outstanding test that might be beneficial

Living Well With COPD – Education and Support Group

This  program is designed to increase understanding of Chronic Obstructive Pulmonary Disease (COPD) and enhance quality of life by teaching how to maintain and improve exercise tolerance. Everyone is welcome to join.

Every Tuesday until April 9, there will be a session in-person,  and streamed on zoom – register for free by clicking here.  You are welcome to attend all sessions or  pick and choose which sessions you want to watch or attend.
In-person sessions are at Ed’s House, 1301 Ontario St. in Cobourg from 1:30—2:30pm.

Sessions will resume in September 2024.
