An exciting opportunity is now available for a new, permanent full-time System Navigator to work in a collaborative model of care in Northumberland County as part of an interdisciplinary team. The successful candidate will assist clinicians and patients in navigating the healthcare system providing personalized support and ensuring high quality, patient-centered care so that patients achieve their healthcare goals. Duties might include, among others: understanding available community resources and how to access them; determining patient needs and where gaps in care lie; coordinating patient care plans and communicating with patients, caregivers, healthcare providers and other community services; facilitating education for patients, caregivers and healthcare providers on available resources locally and beyond; program evaluation and development to ensure efficiency in meeting the needs of our community. NURSE PRACTITIONER – Primary Care Rostering (MRP)

An exciting opportunity is available for *2* Permanent Full Time Nurse Practitioners in Northumberland County, with the Northumberland Family Health Team (NFHT). Nurse Practitioners (NP) are valued members of our team, diagnosing and treating people of all ages. Registered Practical Nurse – Footcare Program
The Northumberland Family Health Team (NFHT) is seeking a skilled and compassionate Registered Practical Nurse (RPN) to join our Footcare Program.
The successful candidate will bring strong clinical assessment, patient education, and organizational skills to support individuals requiring preventative and therapeutic foot care services. The RPN will work collaboratively with the interprofessional team to provide high-quality, patient-centered care focused on maintaining overall foot health, preventing complications such as wounds and infections, and supporting patients experiencing pain, mobility concerns, or chronic health conditions impacting foot health.
This role requires the ability to work independently while also functioning effectively within a collaborative team environment. The RPN will practice within their full scope in the delivery of footcare services, patient education, chronic disease support, and care coordination.
Services are provided in clinic and community settings, supporting patients who may be at increased risk for debility, reduced mobility, ulceration, or infection related to chronic disease and aging.

REGISTERED NURSE for the Home Based Transition Care Team (HBTCT)
Permanent Part Time 0.4 FTE (2 days/week)
The Northumberland Family Health Team (NFHT) is seeking a dynamic and compassionate Registered Nurse (RN) to join our innovative Home-Based Transition Care Team (HBTCT) on a permanent part-time basis (0.4 FTE, 2 days/wk).
The successful candidate will bring strong clinical assessment and critical thinking skills, particularly in the areas of Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), and chronic disease management. This role requires the ability to provide patient-centered education that supports improved health outcomes and empowers patients to effectively self-manage their conditions and symptoms.
Working collaboratively with a Nurse Practitioner and family physicians, the RN will provide home-based care to patients transitioning from hospital to home or those at risk of hospital admission or Emergency Department visits. The RN will use evidence-based clinical practices to assess, monitor, develop, and implement individualized care plans focused on symptom stabilization, patient and caregiver education, and prevention of hospital readmission.
Services begin prior to hospital discharge and continue for approximately 4–8 weeks, concluding with a transition back to the patient’s primary care team.

The successful candidate will be well organized with excellent clinical/critical thinking skills and able to work both independently and within a team environment. Must Haves:
- Bachelors or Master’s Degree in Physiotherapy.
- Current Independent Practice Registration in good standing with the College of Physiotherapists of Ontario.
- Excellent verbal and written communication, collaboration and interpersonal skills with the ability to build and nurture positive working relationships both within and outside of NFHT
- Current Ontario Drivers’ License and access to a reliable vehicle
- Basic CPR certification
You will stand out as a candidate if you also have:
- Previous physiotherapy experience in a primary care and/or interdisciplinary setting
- Previous experience in the area of chronic disease management and prevention, including COPD management and a biopsychosocial approach to chronic pain, in particular.
- Proficiency in the use of computers and electronic medical records systems
- Registration in good standing as a Certified Respiratory Educator (CRE), is an asset.
