NURSE PRACTITIONER – Primary Care Rostering (MRP)
Permanent Full-Time

An exciting opportunity is available for *2* Permanent Full Time Nurse Practitioners in Northumberland County, with the Northumberland Family Health Team (NFHT). Nurse Practitioners (NP) are valued members of our team, diagnosing and treating people of all ages. The NP, acting as the Most Responsible Provider (MRP) for a roster of patients, will provide comprehensive medical care, education and coordination of care for people experiencing a spectrum of medical issues across the lifespan, from new life events to episodic illness to chronic disease. The Nurse Practitioner, and their patients, will be supported by our growing interprofessional team/programs. This position comes with paid vacation, paid time and expenses for professional development, and enrollment in the Healthcare of Ontario Pension Plan (HOOPP).

Must Haves:
• Current registration (or eligible for registration) with the College of Nurses of Ontario as a Nurse Practitioner – Primary Health Care (NP-PHC)
• Graduate of a recognized Primary Health Care Nurse Practitioner Program
• Current membership with RNAO/NPAO, including professional liability insurance program
• Basic CPR certification
• Current Ontario Drivers’ License
• Strong clinical and decision-making skills
• Excellent verbal and written communication, collaboration and interpersonal skills with the ability to build and nurture positive working relationships both within and outside of NFHT

• Experience in community and/or primary care medicine
• Proficient computer skills and experience with Electronic Medical Records (EMRs)

KINESIOLOGIST
Permanent, Full Time

An exciting opportunity is available for a New Permanent, Full-Time Kinesiologist as part of a collaborative, interprofessional team in Cobourg with the Northumberland Family Health Team (NFHT). The successful candidate will provide services to a wide variety of adult patients within the NFHT, with a focus on musculoskeletal injuries, chronic disease management, and the wellness and prevention program. The successful candidate will be well organized with excellent clinical/critical thinking skills and able to work both independently and within a team environment. This position comes with paid vacation and an opportunity to enroll in the Healthcare of Ontario Pension Plan (HOOPP).

  • Current Registration in good standing with the College of Kinesiologists of Ontario.
  • Proficiency in evidence-based rehabilitation for acute and chronic pain, strong foundations in exercise physiology and exercise prescription
  • Excellent verbal and written communication, collaboration and interpersonal skills with the ability to build and nurture positive working relationships both within and outside of NFHT
  • Current Ontario Drivers’ License and access to a reliable vehicle as travel may be required between sites in community.
  • Basic CPR certification

  • Previous experience in a primary care and/or interdisciplinary setting
  • Previous experience in the area of chronic disease management and prevention, including COPD management and a biopsychosocial approach to chronic pain, in particular.

Permanent, Flexible-Time Physiotherapist
