How To Submit Feedback or Complaints
We’d love to hear from you! There are several ways you can provide feedback.
- If you have feedback about any of our programs, services or staff, please speak to a member of our team directly or you can contact us at info@nfht.ca/289-252-2139.
- Your experience matters to us. Please take a few moments to complete our NFHT Patient Experience Survey short survey about your most recent visit with a Northumberland Health Team healthcare provider (such as a physician, nurse, mental health clinician, physiotherapist, occupational therapist, pharmacist, or another team member). This survey is completely anonymous. We do not collect any personal information, and all responses are combined to help us better understand what we’re doing well and where we can improve.
- We work in partnership and collaboration with our Family Physician members. Any feedback or complaints about family physicians must be done through their governing body, the College of Physicians and Surgeons of Ontario, through their complaints section.
