Management of high blood pressure through education, support, healthy lifestyle counseling and evidence based interventions.
This program is for:
- Patients with high blood pressure
Program Activities and Services:
- Individual sessions with care providers for assessment, monitoring, counselling, treatment and self-management support
- Blood pressure checks, lab work monitoring, symptom monitoring
- Group workshops
Goals for Participants:
- Lower blood pressure within individualized target
- Healthy lifestyle behaviour changes such as improved nutrition, increased activity and better stress management
- Improved knowledge, confidence and/or motivation

- Pharmacist
- Nursing
- Dietitian
- Physiotherapist
- Mental Health Clinician
How to Access:
- Physician, Nurse Practitioner or NFHT Clinician referral
