Chronic Obstructive Pulmonary Disease Program
Assessment, treatment, monitoring and support for people with respiratory impairment.
This program is for:
- People with a suspected or confirmed diagnosis of COPD
Program Activities and Services:
- Comprehensive respiratory assessment and/or facilitation of external testing as required
- Health promotion and prevention related to lung health including referral to smoking cessation support, pulmonary rehab and vaccinations as needed
- Development of an Action Plan to support self-management of COPD and acute exacerbations
- Individual and group monitoring and health teaching
Goals for Participants:
- Improved respiratory health and overall quality of life
- Improved confidence and ability to manage COPD and it’s symptoms

- Physiotherapist/Certified Respiratory Educator
- Registered Nurse
- Dietitian
- Pharmacist
- Mental Health Clinician
How to Access:
- Physician, Nurse Practitioner or NFHT Clinician referral
