Chronic Obstructive Pulmonary Disease Program

Assessment, treatment, monitoring and support for people with respiratory impairment.

This program is for: 

  • People with a suspected or confirmed diagnosis of COPD

Program Activities and Services:

  • Comprehensive respiratory assessment and/or facilitation of external testing as required
  • Health promotion and prevention related to lung health including referral to smoking cessation support, pulmonary rehab and vaccinations as needed
  • Development of an Action Plan to support self-management of COPD and acute exacerbations
  • Individual and group monitoring and health teaching

Goals for Participants:

  • Improved respiratory health and overall quality of life
  • Improved confidence and ability to manage COPD and it’s symptoms

  • Physiotherapist/Certified Respiratory Educator
  • Registered Nurse
  • Dietitian
  • Pharmacist
  • Mental Health Clinician

How to Access:

Vaccinations, Masking

We are holding a school age vaccinations clinic on March 15, 2023. To book online click here.

If you have an in-person appointment with a health care provider at any of the NFHT offices, you do still need to wear a procedure mask inside of our office – we can provide one. These infection control protocols are to protect our patients and staff.
