About NFHT

In 2011, the Ministry of Health and Long-Term Care awarded west Northumberland a Family Health Team based on an application submitted by 21 local family doctors and supported by local health care partners. The Northumberland Family Health Team (NFHT) became operational at the end of August, 2011.

The NFHT will form partnerships with other health care providers to ensure that the NFHT patients have seamless and sustainable access to health care in the community, and that joint programs are established to facilitate chronic disease management, health promotion and disease prevention.

All residents of west Northumberland have timely access to exceptional primary health care services through a sustainable health care model.

Our Mission

In an integrated and collaborative manner, the Northumberland Family Health Team will efficiently provide high quality, accessible patient- centered care that offers a comprehensive range of core and targeted services aimed at managing and preventing disease and improving the health status of the population.

If you have an in-person appointment with a health care provider at any of the NFHT offices, please wear your own mask (homemade or otherwise). Please ensure the mask covers both your nose and mouth. These infection control protocols are to protect our patients and staff during the COVID-19 pandemic.

FLU SHOT CLINIC

NFHT is hosting walk-in Flu Shot Clinics every Monday afternoon from 1 to 5pm in the lower level of Northumberland Mall. Starting Oct. 18 with a clinic for those over age 65 or at high risk, for details click here. High Dose is available, pending supply. Clinics will open to the general public in November and there is a Sat. clinic Nov 6 from 9am to 1pm.
