Vascular Health Program

Prevention and management of vascular diseases (including heart disease, stroke, diabetes, kidney or circulatory disease) through self-management support, healthy lifestyle counseling, and evidence-based intervention.

This Program is For:
• Patients with, or at risk for, vascular diseases such as heart disease, stroke, diabetes, kidney disease, circulatory disease

Program Activities and Services:
• Individual sessions with care providers for assessment, monitoring, counseling, treatment, and self-management support
• Blood pressure checks, lab work monitoring, symptom monitoring
• Group workshops

Goals for Participants:
• Improvement in vascular health measures (such as blood pressure, blood sugar, cholesterol, and medications)
• Healthy lifestyle behaviour changes such as improved nutrition and increased activity
• Improved knowledge, confidence, and/or motivation
